Hadir Tepat Waktu / On-time Attendance
Mohon hadir di tempat terapi secara tepat waktu. Sesi terapi individual akan tetap berakhir sesuai dengan waktu yang semula dijadwalkan, meskipun Anda tiba terlambat karena alasan apapun. Bilamana karena suatu hal, terapis Anda terlambat hadir, maka Anda tetap berhak untuk memperoleh terapi dengan durasi yang utuh sesuai perjanjian.

Please attend to your session on time as scheduled. If for any reason you arrive late for your session, the session will still end on its scheduled time, there will be no time extentions. If your therapist arrives late for any reason, you are still entitled to receive a session according to the full duration, so that it could end later than the original schedule.

Ketentuan Pembatalan & Penjadwalan Ulang / Cancellation & Rescheduling Policy
Anda memiliki keleluasaan untuk membatalkan atau menjadwalkan ulang sesi terapi individual Anda tanpa konsekuensi apapun, dengan batas waktu pembatalan / penjadwalan ulang yaitu PADA 1 (SATU) HARI SEBELUMNYA PUKUL 14.00. Setiap pembatalan / penjadwalan ulang dari pihak klien yang dilakukan setelah lewat batas waktu tersebut, akan dikenakan biaya pembatalan/penjadwalan ulang sebesar Rp. 200.000,-. Mohon dimaklumi karena jika slot waktu Anda tidak jadi dipakai, kami pun membutuhkan waktu untuk menggantikan slot tersebut dengan klien lain yang mungkin masih ada di dalam daftar tunggu.

You could cancel or reschedule your session appointment without any consequence, as long as we are notified before the cancellation / rescheduling limit, which is EXACTLY ONE DAY BEFORE YOUR APPOINTMENT AT 14.00. Every cancellation / rescheduling initiated by the client, if notified after the cancellation / rescheduling limit, will be charged a fee of Rp. 200.000,- payable immediately by the client. Pleaes understand that if you do not use the timeslot of your session, we also need time to replace the timeslot with other clients, that may be still on our waiting list.

Pembayaran Tunai saja / Cash Payments Only
Kami hanya menerima pembayaran berbentuk tunai, dan tidak bisa menerima pembayaran melalui kartu kredit, transfer bank, maupun non tunai. Harap persiapkan pembayaran Anda sebelum hadir untuk menjalani terapi. Pembayaran dapat diberikan langsung kepada terapis Anda setelah sesi terapi selesai.

We only accept payments in cash only, and currently unable to receive credit card payments, bank transfer or non-cash payments. Please ensure that you have sufficient cash to settle your session, before arriving for your appointment. Cash payments will be given directly to your respective therapist after your therapy session.

PERNYATAAN KLIEN / RELEASE STATEMENT
Dengan ini saya memberikan izin bagi Reza Gunawan / Nugdha Achadie untuk memberikan terapi penyembuhan holistik (termasuk namun tidak terbatas pada teknik akupunktur, energy psychology, jin shin jyutsu, terapi craniosacral, dan penyembuhan energi) kepada diri saya sesuai tujuan yang dicantumkan dalam formulir ini dan untuk tujuan lainnya yang dapat saya minta di masa depan. Saya memahami bahwa keberhasilan terapi ini sangat bergantung kepada niat dan ketekunan saya sendiri memicu perubahan dalam diri saya. Saya paham bahwa hasil dari terapi saya sangat tergantung pada kesungguhan partisipasi diri saya sendiri sehingga kesuksesan terapi tidak bisa dijamin dalam bentuk apapun. Saya tahu dan paham bahwa Reza Gunawan / Nugdha Achadie akan memberikan upaya yang semaksimal mungkin sebatas kemampuannya untuk memastikan keberhasilan saya.

I hereby authorize Reza Gunawan / Nugdha Achadie to provide me with holistic healing treatments (including but not limited to acupuncture, energy psychology, jin shin jyutsu, craniosacral therapy and energy healing) for the purposes outlined in this intake form and for future purposes that I may request. I understand that the success of my therapy depends greatly on my own intention and commitment to affect change in myself. I understand that because the results of my sessions depend greatly upon my own total participation that the success of the treatment cannot be guaranteed. I am aware however, Reza Gunawan / Nugdha Achadie will do everything in his power to ensure my success.

PERSETUJUAN KLIEN / CLIENT AGREEMENT
Dengan mengklik tombol “setuju” di bawah ini, saya mengerti dan setuju dengan segala ketentuan yang tertera dalam formulir ini. Ketika saya memperoleh konfirmasi dari True Nature Holistic Healing untuk melakukan terapi individual, maka saya juga setuju untuk menandatangani salinan cetak dari formulir online ini, yang akan digunakan sebagai arsip klien individual True Nature Holistic Healing.
